Being an over-achiever, I held myself up to high standards. This set me back when I introduced my first production issue, and I hated myself for it. I eventually come to realize that causing production issues are just part of the development life cycle, and what is important is that we have 1) a good and efficient test plan, 2) understand involved risks and communicate to others, and 3) mitigate potential risk by ensuring that back out plans are ready.
